New Patient Intake

Notice: We have just implemented our Self Service Intake program. IF you have any difficulty, please contact our office. You may be prompted to leave a voicemail when calling in. We appreciate your patience as we launch this new service. 


Welcome to LBH! We are glad that you are taking the first step toward healing and growth. We know this takes a lot of courage.

In order for us to assist you, we need you to complete our intake process. The quickest way to your first appointment is via our New Patient Self Service Intake.  Those who do not have access to an internet enable smartphone, tablet, or computer may call our office to schedule a time to come in to complete the intake process. 


Please follow these simple steps below:

  1. Select the office & provider below that you wish to work with. 

  2. Once you chosen your provider, click the link and you will be prompted to confirm this choice by selecting the option for Self Service Intake. 

  3. You will then be prompted to enter you name, email address and phone number. 

  4. Next you will receive and email invite to register for our Patient Portal powered by Healthie©

  5. Once registered, you will be prompted to complete your intake documents. This process is required. 

  6. Once all required documents are completed you may schedule an appointment with your provider. If there are no appointments available, please reach out via Healthie© Chat or call our office. 

And that's it, you're in!

Please note that our Therapists in Hagerstown currently have a wait list. If you are not sure if you need/want medications, please select one of the NPs.

For those that reside outside of our western Maryland service area, we provide virtual services in the following states: 

Maryland Virtual (Statewide)

*Please note that depending on possible changes with DEA regulations toward the end of 2025, we may not be able to prescribe certain controlled substances in  the future without an in person visit. We do have an administrative office located in Altoona, but our main offices remain in western Maryland. 
**We do not yet accept PA Medicaid.
