Welcome to LBH! We are glad that you are taking the first step toward healing and growth. We know this takes a lot of courage.

Before you get started we encourage you to explore our website as it has a lot of important and helpful information about our practice. Most notable, is that our practice is owned, operated and managed by Psychiatric Nurse Practitioners (PMHNPs). PMHNPs are advance practice nurses that are trained, board certified, and licensed to prescribe medications and provide psychotherapy. In most cases, you will see a PMHNP at our practice, although at times you may have the option to see other providers like professional counselors, social workers, and interns. Our top goal is to provide all of our patients with the best care possible. We look forward to working with you! 

In order for us to assist you, we need you to complete our intake process. Please review the following directions carefully to ensure a smooth intake process. 

Please follow these simple steps below:

1. Click the link below for "Intake Request Form". It will open up a Google Web Form. Please compete this form.

2. Once you submit the webform, our staff will email you an invite from our EMR platform (Healthie). You will need click the link the invite email to finish creating your account and complete all forms required. Please note that this may take up to 2 business days. 

3. Once you have completed all information and the necessary intake forms in Healthie, our staff will review it and verify your insurance information. 

4. After everything is verified, our staff will contact you via phone and/or Portal chat to schedule an appointment for your initial evaluation!
