Welcome to LBH! We are glad that you are taking the first step toward healing and growth. We know this takes a lot of courage.
In order for us to assist you, we need you to complete our intake process. Please follow these simple steps below:
Please complete the form below for our staff to add you to our system.
Once added, you will receive an invite email to our Patient Portal powered by Healthie©.
Once you accept our invitation you can register your portal account.
Once you are registered with the portal, you will be prompted to complete your intake forms.
Once all of your forms are completed, our Intake team will review and verify your insurance.
Once we have verified your insurance, our Intake team will contact you to schedule your first appointment!
